Emergency Relief Program > ERP Residential Enrollment Form

Please complete the Residential Customer Enrollment Form below. Within fifteen (15) business days of receipt, UGI will send an email to you with instructions on how to complete the Emergency Relief Program Agreement electronically. Next, the agreement will be sent through DocuSign to allow you to securely submit the required information and upload supporting eligibility documentation. Instructions to use DocuSign to complete the Emergency Relief Program Agreement will be included in the initial email.

UGI Natural Gas customers in Pennsylvania are eligible for the Emergency Relief Program if a household member:

  • Received unemployment benefits on or after March 13, 2020; or
  • Is eligible for or have received a Federal COVID-19 Relief Check (Federal CARES Act Economic Impact Payment) in the amount of at least $1,200

Note: The name of the household member that has received the above-referenced benefits must be viewable on all documentation provided.

You will be prompted to submit documentation demonstrating one of the above.

Upon receipt of your agreement, UGI will review your submission. If you meet the guidelines for the Emergency Relief Program, you will receive confirmation that you are enrolled in the program.
