To be compliant with the PUC’s Alleged Violation Report (AVR) requirements, UGI has instituted the following process for any 1st or 2nd Party damages:

  1. As soon as possible after ANY damage occurs, 811 is to be notified so a Damage One Call ticket can be generated.
  2. Complete the information below, including as much detail as available and state if the facility was Marked, Mismarked, relative to the tolerance zone, or Unmarked. Include any photos and comments that help describe the situation.
