Emergency Relief Program > ERP Business Enrollment Form

Please complete the Business Customer Enrollment Form below. Be sure to select the NAICS (Principal Business) code that is listed on your federal income tax return, as this is required eligibility documentation. This form should only be used by UGI Natural Gas customers in Pennsylvania with accounts that have a rate classification of N, NT or DS. Within fifteen (15) business days of submitting, UGI will send an email to you with instructions on how to complete the Emergency Relief Program Agreement electronically. Next, the agreement will be sent through DocuSign to allow you to securely submit the required information and upload supporting eligibility documentation. Instructions to use DocuSign to complete the Emergency Relief Program Agreement will be included in the initial email. 

Upon receipt of your agreement, UGI will review your submission. If you meet the guidelines for the Emergency Relief Program, you will receive confirmation that you are enrolled in the program.
