Request a CAP Application By Mail

The Customer Assistance Program (CAP) is an income-based payment plan that offers benefits like past due debt forgiveness.

You can download the CAP application, print it, and send to your local Community Based Organization (CBO). If you do not have access to a computer to print an application, you can request one be mailed to you by filling out the form below.

Please Note: If you currently have an alternate supplier and you want to enroll in the UGI Customer Assistance Program (CAP), you must contact your supplier directly to cancel your Choice contract. Be aware that your supplier may charge you cancellation or other fees to end your Choice contract early. Please contact your supplier directly for details about ending your Choice contract.
