Please complete this part of the form with the information you want listed on UGI’s “Find an HVAC Contractor” webpage. This information will be available to potential customers.

After completing this form, the agreement will be sent for signature via DocuSign. After all paperwork is complete, each employee that completes the online education module will receive a key that will be their individual responsibility. UGI will only send meter lock keys to all individuals who successfully complete the module and knowledge check.

Each employee that will be activating meters must first complete the New Business Activation online education module, which takes about 45 minutes to complete.

List the names of the employees who will be participating in the online education module. Each attendee must have a unique email address to receive the link (which will take less than an hour to complete). Each employee that will be activating meters must complete this education module and knowledge check.
